We will review the scientific evidence on important aspects of clinical decision making such as whether to maintain or extract a periodontally compromised tooth. Data from clinical research with evidence derived from selected clinical cases will be presented to illustrate the process of treatment planning and how and when to extract and replace the tooth with a dental implant.
Data will be presented on the risks associated with implant therapy in periodontally susceptible patients and we shall illustrate with clinical cases how to prevent and how to manage peri-implant infections.
Different surgical protocols will be reviewed, such as the placement of implants in fresh extraction sockets and the immediate functional loading of implants. The biological processes characteristic of both healing situations will be presented and how these principles can be translated to a successful clinical outcome.
Finally, basic principles of the achievement of an aesthetic restorative result will be discussed in relation to the application of surgical approaches to both soft and hard tissues.
